After filling out the form, please email a copy to: conference@fluoridealert.org or mail a printed copy to:

Upon receipt of your registration form and payment, we will send you an information packet with further information about the conference.

PAYMENT OPTIONS:

After completing your registration form, you can choose to pay for your registration fees with a credit card, or with a check.

CREDIT CARD:

Registration fees can be paid online with a credit card at FAN's secure Donation Page.

In order to pay, simply enter the total of your registration fees (as determined in your registration form) into the category titled "Other."

CHECK:

To pay for your registration fees by check, please make check payable to "FAN Conference" and mail (along with the registration form) to:
